What we collect
When you request an appointment, we collect your name, phone number, email address, the service you're interested in, your preferred date and time, and any note you choose to add. When you leave a review, we collect your name, an optional title, your rating and your review.
Please don't send detailed medical information through this website. We'll go over your health history in person, at your consultation.
Why we collect it
- To contact you about your appointment request.
- To publish reviews you've agreed to share, after we've checked them.
- To keep this website secure and working.
We don't sell your information, and we don't use it for marketing unless you've separately agreed to receive marketing messages.
Your consent
Each form asks for your consent before you send it. You can withdraw your consent at any time by contacting us. We'll stop using your information for that purpose, unless we're required by law to keep it.
Where it's stored and how we protect it
The information you send through this website is stored with service providers that keep data in Canada. It's encrypted while it travels to us. Only authorised UniHealth staff can see it, using accounts protected by strong passwords and two-step verification.
Online appointment booking is provided by Jane App. When you book through Jane, Jane's privacy policy also applies.
How long we keep it
Appointment requests that don't become appointments are deleted after 90 days. Information that becomes part of your client record is kept as long as Ontario law requires. Reviews are kept until you ask us to remove them.
Who we share it with
We share your information only with the service providers who help us run this website and our bookings, and only as much as they need. We may also disclose it where the law requires us to.
Your rights
You can ask to see the personal information we hold about you, ask us to correct it, or ask us to delete it where the law allows. We'll respond within 30 days.
This website
This website doesn't use advertising or tracking cookies. If you use the "Pause animations" button, your browser remembers that choice on your own device. Nothing about it is sent to us.
